New Patient

• Please bring your insurance card and the script written by your doctor (if you have one). If you do not have insurance, a prescription from a doctor is helpful but not required.
  
  
• Please download the attached five forms and have them filled out when you arrive for your fist appointment OR arrive 10-15 minutes early to fill out this paperwork in our office, you must complete all paperwork before your first visit.
  
  
• Please dress appropriately (if you are being seen for a knee injury please wear or bring shorts)
  
  
• Plan on a one hour appointment.
